Helping chronically ill teenagers transition to better health care

Chronically ill teenagers face a critical transition as they move from paediatric care — often warm, supportive, and child focused — into adult healthcare systems that lack tailored support for this age group. Monash Hospital wanted to design a better experience for teenagers and carers navigating this shift.

We led a series of co-design sessions with patients, carers, nurses, doctors, and hospital staff to uncover unmet needs and explore new service and digital solutions that could support young people more holistically.

Outcomes

  • Facilitated co-design sessions with teenagers, carers, and clinicians
  • Built a business case to progress concepts beyond pilot stage
  • Presented outcomes to the hospital board, securing $300k funding
  • Launched Monash Watch, a digital tool connecting patients and clinicians
  • Sparked the creation of Isobar Good, dedicated to social impact work
